- GLP-1 receptor agonist only — not GIP, not glucagon. Titration is the protocol: 0.25 → 0.5 → 1.0 → 1.7 → 2.4 mg weekly.
- GI is dose-limiting. Nausea, early satiety, constipation. If the gut is still loud at week 4, hold the step. Do not chase the 2.4 mg ceiling.
- Appetite drop is steep. Lean-mass loss follows unless protein and resistance work stay in the protocol. Scale weight is not the only readout.
